The purpose of the webinar is to provide an overview of local services that offer education to patients with diabetes and pre-diabetes and local weight management services that are available to support patients with their weight loss. You will get to know:
- The referral criteria and how to refer into each service
- What a patient can expect once they are referred into each service, i.e. individual support, group support, face to face & virtual offers, how long the patient will be supported by the service and what they can expect after completing the programmes
- How we make our services accessible to those with specific needs, e.g. interpreter services, easy read materials, bespoke interventions/materials for patients from different cultural backgrounds.
- Q&A’s
At the end of the webinar you will have a good knowledge and understanding of each service allowing you to support your patients in making an informed decision on which service is the most appropriate for their individual needs at that point.
