Advance Care Planning:
The purpose of the session is to promote the benefits of and improve processes for advance care planning. The importance of having meaningful conversations will run alongside this session.
The session will consider:
-What advance care planning is and what it means for patients, carers and staff
- How values, wishes and beliefs can be communicated and the different forms of ACP
- How to put ACP into practice and the importance of sharing this information effectively
