In Britain today, six out of ten adults are living with a long-term condition that cannot currently be cured. With these conditions accounting for nearly 80 per cent of GP consultations, it’s perhaps no surprise that primary care teams are keen to improve their knowledge and skills when it comes to supporting their patients with long-term needs. Add to this the numerous new roles being introduced into primary care, and we know that there is a significant education and upskilling job to be done.
To meet the needs of patients with long-term conditions, we know that offering patients holistic care pathways – which begin in General Practice – is increasingly important. At Health Education Derbyshire (HED), we want clinicians in Derby and Derbyshire to be able to offer personalised care and support, in which the person with a long-term condition is an active and equal partner.
Being able to support patients right from the very start of their journey, in a setting where they’re comfortable, undoubtedly improves outcomes and allows clinicians to deliver better care. At the level of the individual, health and social care professionals must be able have more productive conversations with patients, focussed on what matters to them and are able to play a much bigger role in planning their own care. These changed conversations do not happen on their own or by luck. Plenty of help is needed, not least from educational and training origination like HED.
This Diabetes Week (13th-19th June), we want to provide our colleagues in Primary Care with a full run-down of the support and guidance we offer around better diabetes care. Through a range of more than twenty courses and resources, we can help clinicians learn about diabetes guidelines, oral and injectable agents, case reviews, foot care, continuous blood glucose monitoring, lifestyle services, carbohydrate awareness, consultation skills and much more.
The good news is you can study at a time and place that suits you - via face-to-face or online accredited sessions, recorded videos and accredited diabetes modules with post course access to mentorship from your local diabetes team.

A large part of our offering are online and face-to-face sessions which aim to provide information and training at various levels. With something suitable for anyone working in General Practice, our range of masterclasses include Achieving diabetes Care Excellence, ADEPT Diabetes Training and Getting Diagnoses Right. We understand the importance of interactive learning and have created a suite of facilitated workshops through a model of interactive blended online learning which includes videos, live Q&A sessions, and opportunities to share best practice with peers. There are a range of refresher courses available too, if you’re keen to go over subjects you’ve covered in the past.
We also offer a range of short courses taught at Level 5 (diploma level) as well as courses taught at Levels 6 and 7 too. And, knowing how much participants benefit from interactive sessions with a trainer, we have created an interactive blended online learning including videos and Q&A sessions to ensure you get the opportunity to ask questions and discuss with peers.
And, finally, we have several easily digestible resources which you can share with your patients as well as using to increase your own knowledge and expertise. Videos and guides include: Carb Awareness Video, a guide to Weight Management Services and low calorie diet pilot training for GPs, pharmacists, nurses and pharmacy techs.
